Need A Sample For Proof Of Ressidence

Hello,
I need help urgently please.
I have been approached by one of my staff requesting that i write him a froof of residence letter from the company so he can use it in the bank.
I an kindly asking if there is anyone who knows how to design/write something like that to help me.
