5. Register of Deduction Loss/Damage (Form: XX)
6. Register of Fines (Form: XXI)
7. Register of Advances (Form: XXII)
8. Register of Overtime (Form: XXIII)
9. Wage slips (Form: XIX)
10. Register of Workmen (Form: XIII)
11. Employment card (Form: XIV)
12. Service certificate (Form: XV)
13. Time in & out Register
14. Letter of Appointment (Form: Q)
15. Leave card (Form: H)
16. Weekly Off
17. Weather Leave with Wage Register maintained
18. Weather Leave with Wage is extended to all the employees deployed, EL-1@20 days worked & SL/CL-12 days
19. Accident Register in Form: 11 is to be maintained in the establishment, and reports of accidents in Form: 16 to be maintained by contractor/employer
20. EPF Registration Certificate
21. Remittance of the contributions before 15th of every month
22. Whether copies of Form 12A, 5 & 10 before 21st of every month
23. ESI Registration Certificate
24. Whether contributions remitted through challans in SBI by the 21st of every month
25. Employees who are drawing less than Rs. 15,000/- PM are to be covered under the ESI Act by obtaining the particulars of employees in Form: 1 and submitting the same to ESIC with a photocopy for obtaining an ESI card
26. Whether Minimum Wage Paid
27. Every employee should be allowed 5 national holidays & 5 festival holidays
28. The statement of national & festival holidays to be sent to the Labour Inspector and the same copy to be displayed on the notice board
29. If any employee works on national & festival holidays, whether additional payment/compensatory off made
30. Overtime working Yes/No, rates of payment
31. Overtime paid shown in the Overtime Register & ESI Contribution
32. Whether bonus is paid to all employees who have completed at least 30 days of service with the contractor
33. Whether bonus is paid to all those deployed in the company in the presence of the representative of the company and endorsed in Form: C
34. Whether bonus Registers Form: A & B are maintained
35. Whether annual return in Form D submitted within a month of payment of Bonus
36. Labour Welfare Fund Certificate
37. PT Registration Certificate
