Questionaire Require For Organization Performance Measurement Using BSC

some one can send me the questionaire related to the measure the organizational performance using balance score card. i will appriciate if some one can send me the questionaire today because tomorrow is the last date for submission of my proposal for final project.(MBA-HRM)

Dear i got reply from my teacher on 15th of June and last date of submission was given there 18th what can i do only in 3 days while i am on job and having to much stress there now a days.
