Questionnaire On Effectiveness Of Performance Management

Hello members and senior members,
my name is Dhanya and im doin my project in HR at Tata Teleservices. I've chosen "Performance Management System" as my topic for the internship. Im planning to do a questionnaire on the effectiveness of Performance Management System at TTSL. I would be really grateful if somebody helps me out with the questionnaire and analysis. Could somebody suggest some good idea as to how I could go about the whole thing...
