Please guide me ,
Employee joined in our organisation. He had ESI Card in his previous Organisation.
Please let me know what is the procedure to continue the same card/Insurance number in our organisation.
What are Forms required?

No procedure only you have to remit the contribution of "That " employee to his ESI number.

He has to fill up declaration form indicating his Insurance no. in the form and submit to your Local office to make necessary amendments in the ESI card for Change in employment and also to transfer his records from Previous Local ESI office to your present Local ESI office.

Hi Prasad,
There is some process is thier. The emp must fill up Form 53. In there you have to See Other changes: you put Employer change (your emplr code) then give it your purticular ESI Branch office.
