Calculating Monthly Conveyance

Hi Friends,
Can anyone send me format for calculating monthly conveyance for staff, if any one use his own / company vehicle (2/3/4 Wheelers).

In our Company for official tour we do pay
For Car - Rs. 6 / km ( Subject to Car Eligibility & prior approved by HoD/ GM )
For 2 Wheeler -Rs. 2.5 /km ( Subject to Car Eligibility & prior approved by HoD/ GM )

Thanks prashant, do u hv any format. Although, it simple but still i want if there any format available with anyone so that we can put max. Inf. On that format. Or let's all prepare a formats and share with each other and make a final-one.
