QUESTIONNAIRE FOR HOTEL STAFF

Dear all,
Please help me out to draft a questionnaire to interview the cooks,waiters,F& B managers, Chef etc..
Also we are facing problems in store management in our Restaurants.
Kindly help me out in posting a good format that can be implemented in stores where there is huge turnover in materials for our 10 outlets.
