Considering that your unit is covered under The Maternity Benefit act, 1961 & you are entitle to have have the benefit of Maternity leaves from the employer you are hereby advised that you are to give notice under section 6 of this Act to your employer in the prescribed format along with the the medical certificate in the prescribed format as given in your State Maternity Benefit Rules where you are working.
