The only hassle with your format is that the observation is not objective enough. We could consider quantifying each attribute by awarding marks. However, the maximum marks for each attribute have to be determined, which would then total up to an overall score (possibly out of 100). This is just an idea. You could also compare teachers' performances and grade them within the staff in order of merit.

Thank you for your valued suggestion. Actually, this observation is a part of the program where a teacher conducts self-appraisal. It is meant for qualitative assessment. However, when the same observation report is used for evaluating a teacher for promotion or a salary increase, it needs to be quantitative. Anyway, let me also make it quantitative and send it across to you. I appreciate you once again for reviewing my report and providing your feedback.
