The format sent by Mr. Rajesh is really suited to all the needs to appraise an executive or manager. However, there may be some variations depending on the type of industry, for example, service, manufacturing, IT, etc., and the kind of job the executive handles, such as office job (independent, no subordinates), stationed job, etc. Some weightage can be given based on the required traits, known as competency-based appraisals.
