ESIC-Med. (Com) - 11

Dear Sir/Madam,
Please tell me the what is the purpose of ESIC-Med. (Com) - 11 form becoz i got such form one of our employee and i have no idea about this. Please help.

For information to the employer to regulate leave and make alternate arrangements, issue for the corresponding period from a separate book along with Forms 11, 21, and 23. Also issue a duplicate of a lost certificate for information to the employer, quarantine, and court purposes.
