You have not mentioned the name of your appropriate Govt. In Haryana State we are not required to prepare any covering letter as we are to apply for obtaining factory licence/renewal of licence by online system on Haryana Labour department web-site.

i have already submit factory renewal license before dec 2019, but pending till date reason.
