Employee Name Change

Request for Name change has to be dealt in a systematic manner since it has wide ramifications.
The concerned employee has to submit a Affidavit. He should get it published in a local news paper and finally Gazette Notification is must.
He has to submit all the above three documents for his name change

you need to a copy of name change in newspaper & attested copy of name change through court.

nukani is right, you should ask gazet copy. Then only the employee get his future benefits from employer.
If he not applied for that, ask him publish and get a copy.

He needs to get Name Change Affidavit on Stamp Paper duly attested by Public Notary. The photocopy of the same will suffice to amend the data base & other records at your organisation.

Yes definitely you need to get the proof. Tell him to wait till he receives all the necessary documents for the change of name.
