Employee Pension Scheme Certificate

Respected sir,
I worked in Pvt. Org. since 01.10.2009 to 30.6.2017 and left for 3 months again rejoined in same establishment on 01.10.2017 to 30.04.2018. Just I left in May 2018.... Pls. suggest what should I do... there is no scope to continue my job at present org. My age is 52 + (01.07.1966)..
pls. suggest....
