Need Help with ESIC Form 37? What Other Forms Should I Be Aware Of?

Please guide me on how to fill out Form 37 of ESIC. Also, let me know if there are any other forms that need to be filled out in connection to this.

The Form 37 of the ESIC is self-explanatory. Fill up the details of the employee/ESI number, but take care that while submitting the same to the local office, your office should be in their jurisdiction. Also, if there is an ESI number of the employee, please mention it.

Dear Amit,

Form 37 is a declaration by the employer regarding the continuity of the employee during the insurable contribution period to be submitted upon demand by the local ESI office.

In ESI Form 37, only four requirements are needed:
1. Name & Address along with ESI Code of the Employer.
2. Name and Insurance number of the employee.
3. Declaration of the start date of the current contribution period and the date of reemployment/reentering the insurable employment of the employee.
4. Declaration of payment for not less than half of the number of days worked during the last half-yearly return which ended on 31st March or 30th September.

It is a simple process. Fill it out and submit the form to the Local ESI office.

Please send me a filled sample of Form 37. What is the meaning of paragraphs 1 and 2 of Form 37? Also, please provide the declaration start date of the current contribution period and information on reemployment/reentering of insurable employment. I am unable to understand this date.

We have registered the ESIC establishment code for Gurgaon, and our local office is in Mumbai. We need to confirm whether employees can avail benefits using the same code. However, the local office has informed us that the details are not visible. Could you please clarify if employees are eligible for benefits under this code?
