One of my employees has completed treatment in a private hospital and is now approaching the ESI for the claim process. They are asking for a letter from us (the employer) stating that no funds have been given to the employee for treatment.
Could you please assist me with how to write a formal letter or provide any format for this letter?

The employees have the option of taking treatment at a private hospital only if the particular treatment option is not available at the ESI hospital. Is the employee undergoing surgery or just receiving medical treatment for an illness?
