Want to correct the Dipti Srivastava - Employee Contribution goes only in EPF (12% from his salary) whereas Employer Contribution 12% goes in EPS (8.33%) and in EPF (3.67%), and the rest 01% goes in EPF admin charges and EDLI. Now, coming to your attachment:

01. Claim 1 is for EPF withdrawal, where it was rejected due to your name not being printed on the attached cancel cheque. Please file it again with a cheque that has your name printed on it.
02. Claim 2 is for EPF transfer, and I cannot provide the exact reason until form 13 is displayed.
