Dear Friend,
As requested by you , I am attaching the P.F Re-authorisation form. Please note that you have to change the address of the form on top according to your area P.F office, because this form is based on Mumbai. Apart from this, we can use the same across the India.

Hello,
I have been asked to submit the re-authorization form for the as name in bank is not expanded and in my of account it is expanded.
I googled the form and got it your website. If its the right form can you please guide me in
1) What details need to fill in reference section (e.g. intimation No.)
2) what need to be fiilled in the sentence "AG under copy to the undersigned".
Also let me know if you know long does it take to refund after Re-authorization process. Awaiting your response.

What details need to fill in reference section (eg. intimation No.)
What need to be filled in the sentence "AG under copy to the undersigned".

Hello,
Even I am facing an issue more or less same as above. I have submitted the PF form and now it is under process. The bank A/c I have referred has got inactivated 2 weeks back. Now I want to give the different bank A/C. All my PF details are maintained in Mumbai office (Employer and PF office).
In this case can I send the reauthorization form through courier to my old employer asking them to attest and can I send that back to PF office? Does that works?

Now, my salary account has been completely closed. I have a new account with CITIBANK, and the details are entirely different, including my mobile number, name, and address compared to the previous company and PF office. Is it possible for me to update my details with the PF office? I want to transfer the PF amount to my new active account. How can I go about changing my details with the PF office?
